I love these! They are called Bag in a Box, and the tutorial has been around for a few years. I've made them before, but completely forgot about them. If you would like to watch a YouTube tutorial, HERE is the one I followed. 

You'll need the We R Memory Keepers Envelope Punch Board.
Basically you start off with 6" x 6" double sided cardstock.
Place paper edge at 1 3/8", punch and score. 
Slide paper down to 4", punch and score.
Then turn it to the other 3 sides lining up the score lines with the guide, punch and score.
Round your corners.
On the 2 sides with the larger triangle, you'll cut down both sides of that to the score line. This allows you to fold those sides.
It's so cute!!

I used Close To My Heart cardstock and sponged all the fold lines with ink.
